Request for funding must be submitted in writing via mail, fax, or email and contain the following:

• Name, address, phone number, charitable registration number or appropriate charitable designation, list of executives and board of directors, and the most recent audited financial statements of the submitting organization
• Organization profile and overview of the program including:
  • Objectives
  • Geographical area served
  • Target group of people who will benefit and how they will benefit
  • How the project addresses needs that are not already being met by other programs
  • How program results are measured
  • Amount of funding requested
  • Names of Provident employees involved with the organization
  • Details on how Provident's participation and/or sponsorship will be profiled in the community or with the project
